Mechanism Of Action Of Pioglitazone & Glimepiride
- Pioglitazone is a selective agonist at peroxisome proliferator-activated receptor-gamma (PPARγ) in target tissues for insulin action such as adipose tissue, skeletal muscle, and liver.4,2 Activation of PPARγ increases the transcription of insulin-responsive genes involved in the control of glucose and lipid production, transport, and utilization.
- Through this mechanism, pioglitazone both enhances tissue sensitivity to insulin and reduces the hepatic production of glucose
- Like all sulfonylureas, glimepiride acts as an insulin secretagogue. It lowers blood sugar by stimulating the release of insulin by pancreatic beta cells and by inducing increased activity of intracellular insulin receptors
Therapeutic Indications Of Duetact 30/4 Tablets
- It is used to lower blood sugar levels in patients with diabetes mellitus type 2 (non-insulin-dependent). It is used when diet and exercising have failed to control your diabetes
Method Of Administration Of Duetact 30/4 Tablets
- Take with the first meal of the day.
- For patients switching from a different sulfonylurea, carefully monitor for hypoglycemia for 1 to 2 weeks due to a potential overlapping drug effect.
- Individualize dose based on safety, efficacy, and prior therapy.
- Initial dose: Pioglitazone 30 mg - Glimepiride 2 mg or Pioglitazone 30 mg-Glimepiride 4 mg orally once daily.
- Maintenance Dose: gradually titrate dose based on the adequacy of therapeutic response and tolerability for each component.
- Doses may be changed in special conditions so always follow your doctor's or pharmacist's instructions.
- If you take more than you should, talk to a doctor or go to the hospital straight away.
- If you forget to take a dose, take it as soon as you remember it unless it is nearly time for your next dose.

Normal Ranges
- Random blood sugar test: A blood sample will be taken at a random time. Regardless of when you last ate, a blood sugar level of 200 milligrams per deciliter (mg/dL) or higher suggests diabetes. A blood sugar level of less than 140 mg/dL is normal. A reading between 140 and 199 mg/dL indicates prediabetes
- Fasting blood sugar test. A blood sample will be taken after an overnight fast. A fasting blood sugar level of less than 100 mg/dl is normal. A fasting blood sugar level from 100 to 125 mg/dL is considered prediabetes. If it's 126 mg/dL or higher on two separate tests, you may have diabetes.
