Mechanism Of Action Of Sacubitril & Valsartan

  • Entresto, a first-in-class angiotensin receptor-neprilysin inhibitor, has a dual mode of action that simultaneously inhibits vasoconstriction and promotes vasodilation.
  • Entresto relaxes blood vessels, improves blood flow, and reduces stress on the heart.

 

Therapeutic Indications Of Entresto 200 Mg Tablets

  • It is used to treat a type of long-term heart failure in adults. This type of heart failure occurs when the heart is weak and cannot pump enough blood to the lungs and the rest of the body.
  • The most common symptoms of heart failure are breathlessness, fatigue, tiredness, and ankle swelling

 

Method Of Administration Of Entresto 200 Mg Tablets

  • Swallow the tablets with a glass of water. You can take Sacubitril/Valsartan with or without food.
  • You will usually start by taking 24 mg/26 mg or 49 mg/51 mg twice a day (one tablet in the morning and one tablet in the evening). Your doctor will decide your exact starting dose based on which medicines you have been taking previously.
  • Your doctor will then adjust the dose depending on how you respond to the treatment until the best dose for you is found.
  • The usual recommended target dose is 97 mg/103 mg twice a day (one tablet in the morning and one tablet in the evening).
  • Patients taking Sacubitril/Valsartan can develop low blood pressure (dizziness, lightheadedness), a high level of potassium in the blood (which would be detected when your doctor performed a blood test), or decreased kidney function. If this happens, your doctor may reduce the dose of any other medicine you are taking, temporarily reduce your Sacubitril/Valsartan dose, or stop your Sacubitril/Valsartan treatment completely.
  • Doses may be changed in special conditions so always follow your doctor's or pharmacist's instructions.
  • If you take more than you should, talk to a doctor or go to the hospital straight away.
  • If you forget to take a dose, take it as soon as you remember it unless it is nearly time for your next dose.
  • Do not take a double dose (two doses at the same time) to make up for the forgotten dose.

 

Undesirable Side Effects Of Entresto 200 Mg Tablets

  • Although the following side effects are not common in everyone, some patients may complain from:
  1. Low blood pressure (dizziness, light-headedness)
  2. High level of potassium in the blood (shown in a blood test)
  3. Decreased renal function (renal impairment)
  4. Cough
  5. Dizziness
  6. Diarrhea

 

Pregnancy & Breastfeeding Precautions While Taking Entresto 200 Mg Tablets

  • If you are pregnant or breastfeeding, think you may be pregnant or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.
  • Your doctor will discuss the possible risks and benefits of using this medicine during pregnancy and breast-feeding

 

Non-Pharmacological Treatments & Advice For Patients With Heart Failure

  • Weight yourself continuously, if you do experience sudden weight gain, check your ankles, legs, and abdomen for swelling. This could be a sign of edema or the buildup of fluid in your tissues
  • Monitor your blood pressure at home using for example Beurer, Blood Pressure Monitor or Omron Blood Pressure Measuring Device
  • Downsize Salt Intake
  • Take Your Medication as Directed

 

Warnings & Special Precautions while Taking Entresto 200 Mg Tablets

  • Do not take Sacubitril/Valsartan:
  1. if you are allergic to sacubitril, valsartan, or any of the other ingredients of this medicine
  2. if you are taking another type of medicine called an angiotensin-converting enzyme (ACE) inhibitor (for example enalapril, lisinopril, or ramipril). ACE inhibitors are used to treat high blood pressure or heart failure. If you have been taking an ACE inhibitor, wait for 36 hours after taking the last dose before you start to take this medicine.
  3. if you have diabetes or impaired kidney function and you are being treated with a blood pressure-lowering medicine containing aliskiren
  4. if you have severe liver disease.
  5. if you are more than 3 months pregnant (it is also better to avoid this medicine in early pregnancy).
  • Talk to your doctor or pharmacist before taking Sacubitril/Valsartan:
  1. if you are being treated with an angiotensin receptor blocker (ARB) or aliskiren.
  2. if you have ever had angioedema (swelling of the face, lips, tongue, and/or throat, which may cause difficulties in breathing or swallowing).
  3. if you have low blood pressure or are taking any other medicines that reduce your blood pressure (for example, a diuretic) or are suffering from vomiting or diarrhea, especially if you are aged 65 years or more, or if you have a kidney disease and low blood pressure.
  4. if you have severe kidney disease.
  5. if you are suffering from dehydration.
  6. if your kidney artery has narrowed.
  7. if you have liver disease.
  • Tell your doctor or pharmacist if you are taking, have recently taken, or might take any other medications including over-the-counter non-prescription medicines, prescription medicines, herbal supplements, vitamins, and supplements.

 

Storage Conditions Of Entresto 200 Mg Tablets

  • Store below 25°c
  • Don’t refrigerate
  • Keep this medicine out of the sight and reach of children.
  • Store in the original package to protect from moisture.
  • Do not use this medicine after the expiry date which is stated on the box/blisters/label. The expiry date refers to the last day of the month.

 

Normal Ranges

  • Normal: Blood pressure numbers of less than 120/80 mm Hg
  • Elevated: is when readings consistently range from 120-129 systolic and less than 80 mm Hg diastolic.
  • Hypertension Stage 1: is when blood pressure consistently ranges from 130-139 systolic or 80-89 mm Hg diastolic.
  • Hypertension Stage 2: is when blood pressure consistently ranges at 140/90 mm Hg or higher.
  • Hypertensive crisis: This stage of high blood pressure requires medical attention. If your blood pressure readings suddenly exceed 180/120 mm Hg, wait five minutes and then test your blood pressure again. If your readings are still unusually high, contact your doctor immediately. You could be experiencing a hypertensive crisis.
