Mechanism Of Action Of Ramipril
- Ramipril belongs to Angiotensin-converting-enzyme inhibitors which are a class of medication used primarily for the treatment of high blood pressure and heart failure. They work by causing relaxation of blood vessels as well as a decrease in blood volume, which leads to lower blood pressure and decreased oxygen demand from the heart.
- In addition, Ramipril blocks the conversion of angiotensin I to angiotensin II and prevents the degradation of vasodilator prostaglandins, thereby inhibiting systemic vasoconstriction, regulated by angiotensin II, and promoting systemic vasodilation.
Therapeutic Indications Of Ramipril Sandoz 5 Mg Tablets
- To treat high blood pressure (hypertension): high blood pressure is a common condition in which the long-term force of the blood against your artery walls is high enough that it may eventually cause health problems, such as heart disease.
- To reduce the risk of you having a heart attack or stroke
- To reduce the risk or delay the worsening of kidney problems
- To treat your heart when it cannot pump enough blood to the rest of your body (heart failure)
- Treatment following a heart attack (myocardial infarction) is complicated with heart failure.
Method Of Administration Of Ramipril Sandoz 5 Mg Tablets
- Always take this medicine exactly as your doctor has told you. Check with your doctor or pharmacist if you are not sure.
- Swallow the tablet with a drink of water.
- Do not crush or chew the tablets.
- Treatment of high blood pressure:
- The usual starting dose is 1.25 mg or 2.5 mg once daily. Your doctor will adjust the amount you take until your blood pressure is controlled.
- The maximum dose is 10 mg once daily.
- If you are already taking diuretics (water tablets), your doctor may stop or reduce the amount of the diuretic you take before beginning treatment with ramipril.
- To reduce the risk of you having a heart attack or stroke:
- The usual starting dose is 2.5 mg once daily. Your doctor may then decide to increase the amount you take.
- The usual dose is 10 mg once daily.
- Treatment to reduce or delay the worsening of kidney problems:
- You may be started on a dose of 1.25 mg or 2.5 mg once daily. Your doctor will adjust the amount you are taking.
- The usual dose is 5 mg or 10 mg once daily.
- Treatment of heart failure:
- The usual starting dose is 1.25 mg once daily. Your doctor will adjust the amount you take.
- The maximum dose is 10 mg daily.
- Two administrations per day are preferable.
- Treatment after you have had a heart attack:
- The usual starting dose is 1.25 mg once daily to 2.5 mg twice daily. Your doctor will adjust the amount you take.
- The usual dose is 10 mg daily.

- Doses may be changed in special conditions so always follow your doctor's or pharmacist's instructions.
- If you take more than you should, talk to a doctor or go to the hospital straight away.
- If you forget to take a dose, take it as soon as you remember it unless it is nearly time for your next dose.
- Do not take a double dose (two doses at the same time) to make up for the forgotten dose.
Undesirable Side Effects Of Ramipril Sandoz 5 Mg Tablets
- Headache or feeling tired
- Feeling dizzy. This is more likely to happen when you start taking Ramipril or start taking a higher dose
- Fainting, hypotension (abnormally low blood pressure), especially when you stand or sit up quickly
- Dry tickly cough, inflammation of your sinuses (sinusitis) or bronchitis, shortness of breath
- Stomach or gut pain, diarrhea, indigestion, feeling, or being sick
- Skin rash with or without raised area
- Chest pain
- Cramps or pain in your muscles
- Blood tests show more potassium than usual in your blood.
- Balance problems (vertigo)
- Itching and unusual skin sensations such as numbness, tingling, pricking, burning, or creeping on your skin (paresthesia)
- Loss or change in the way things taste
- Sleep problems
- Feeling depressed, anxious, more nervous than usual, or restless
- Blocked nose, difficulty breathing, or worsening of asthma
- Swelling in your gut which is called “intestinal angioedema” presenting with symptoms like abdominal pain, vomiting, and diarrhea.
Pregnancy & Breastfeeding Precautions While Taking Ramipril Sandoz 5 Mg Tablets
- If you are pregnant or breastfeeding, think you may be pregnant or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.
Non-Pharmacological Treatments & Advice To Reduce Blood Pressure
- To measure blood pressure correctly:
- Don’t eat or drink anything 30 minutes before you take your blood pressure.
- Empty your bladder before your reading.
- Sit in a comfortable chair with your back supported for at least 5 minutes before your reading.
- Put both feet flat on the ground and keep your legs uncrossed.
- Rest your arm with the cuff on a table at chest height.
- Make sure the blood pressure cuff is snug but not too tight. The cuff should be against your bare skin, not over clothing.
- Do not talk while your blood pressure is being measured.

Warnings & Special Precautions while Taking Ramipril Sandoz 5 Mg Tablets
- Talk to your doctor or pharmacist before taking Ramipril:
- If you have heart, liver, or kidney problems
- If you have lost a lot of body salts or fluids through being sick (vomiting), having diarrhea, sweating more than usual, being on a low salt diet, taking diuretics for a long time
- If you are going to have treatment to reduce your allergy to bee or wasp stings
- If you are going to receive an anesthetic.
- If you have high amounts of potassium in your blood (shown in blood test results)
- If you are taking medicines or have conditions that may decrease sodium levels in your blood.
- If you are taking medicines that may increase the risk of angioedema, such as mTOR inhibitors, vildagliptin, neprilysin (NEP) inhibitors, or sacubitril/valsartan.
- If you have a collagen vascular disease such as scleroderma or systemic lupus erythematosus.
- You must tell your doctor if you think that you are (or might become) pregnant. Ramipril is not recommended in the first 3 months of pregnancy and may cause serious harm to your baby after 3 months of pregnancy.
- If you are taking any of the following medicines used to treat high blood pressure: an angiotensin II receptor blocker (ARBs), aliskiren.
- Tell your doctor if you are taking any of the following medicines. They can make Ramipril work less well:
- Medicines used to relieve pain and inflammation such as ibuprofen or indometacin
- Medicines used for the treatment of low blood pressure, shock, cardiac failure, asthma, or allergies such as ephedrine, noradrenaline, or adrenaline.
- Tell your doctor if you are taking any of the following medicines. They can increase the chance of getting side effects if you take them with Ramipril:
- Sacubitril/valsartan – used for treating a type of long-term (chronic) heart failure in adults.
- Medicines used to relieve pain and inflammation (e.g. Non-Steroidal Anti- Inflammatory
- Drugs (NSAIDs) such as ibuprofen or indometacin and aspirin)
- Medicines for cancer (chemotherapy)
- Medicines to stop the rejection of organs after a transplant such as a ciclosporin
- Diuretics (water tablets) such as furosemide
- Medicines that can increase the amount of potassium in your blood such as spironolactone, triamterene, amiloride, potassium salts
- Steroid medicines such as prednisolone
- Allopurinol (used to lower the uric acid in your blood)
- Procainamide (for heart rhythm problems)
- Temsirolimus (for cancer)
Storage Conditions Of Ramipril Sandoz 5 Mg Tablets
- Store below 25°c
- Don’t refrigerate
- Keep this medicine out of the sight and reach of children.
- Store in the original package to protect from moisture.
- Do not use this medicine after the expiry date which is stated on the box/blisters/label. The expiry date refers to the last day of the month.
Normal Range
- Normal: Blood pressure numbers of less than 120/80 mm Hg
- Elevated: is when readings consistently range from 120-129 systolic and less than 80 mm Hg diastolic.
- Hypertension Stage 1: is when blood pressure consistently ranges from 130-139 systolic or 80-89 mm Hg diastolic.
- Hypertension Stage 2: is when blood pressure consistently ranges at 140/90 mm Hg or higher.
- Hypertensive crisis: This stage of high blood pressure requires medical attention. If your blood pressure readings suddenly exceed 180/120 mm Hg, wait five minutes and then test your blood pressure again. If your readings are still unusually high, contact your doctor immediately. You could be experiencing a hypertensive crisis.
