Mechanism Of Action Of Perindopril & Indapamide

  • Perindopril belongs to Angiotensin-converting-enzyme inhibitors which are a class of medication used primarily for the treatment of high blood pressure and heart failure. They work by causing relaxation of blood vessels as well as a decrease in blood volume, which leads to lower blood pressure and decreased oxygen demand from the heart.
  • In addition, Perindopril blocks the conversion of angiotensin I to angiotensin II and prevents the degradation of vasodilator prostaglandins, thereby inhibiting systemic vasoconstriction, regulated by angiotensin II, and promoting systemic vasodilation.
  • Indapamide has a dual mechanism of action: diuretic effect at the level of the distal tubule in the kidney and a direct vascular effect, both of which contribute to the antihypertensive efficacy of the drug. The SR formulation contains a hydrophilic matrix, which delivers a smoother pharmacokinetic profile.

 

Therapeutic Indications Of Preterax Tablets

  • It is indicated in patients whose blood pressure is not adequately controlled on Perindopril or Indapamide monotherapy
  • High blood pressure is a common condition in which the long-term force of the blood against your artery walls is high enough that it may eventually cause health problems, such as heart disease.
  • Lowering blood pressure reduces the risk of fatal and nonfatal cardiovascular events, primarily strokes and myocardial infarctions.

 

Method Of Administration Of Preterax Tablets

  • Take exactly as prescribed by your doctor or your pharmacist has told you. Check with your doctor or pharmacist if you are not sure.
  • Take a tablet before food in the morning.
  • Take a tablet with a full glass of water.
  • The recommended dose is one tablet orally once daily. Your doctor may increase the dose to 2 tablets every day or change the dosage regimen if you have kidney dysfunction
  • The use of this medicine is not recommended in children and adolescents.
  • Doses may be changed in special conditions so always follow your doctor's or pharmacist's instructions.
  • If you take more than you should, talk to a doctor or go to the hospital straight away.
  • If you forget to take a dose, take it as soon as you remember it unless it is nearly time for your next dose.
  • Do not take a double dose (two doses at the same time) to make up for the forgotten dose.

 

Undesirable Side Effects Of Preterax Tablets

  • Although the following side effects are not common in everyone, some patients may complain from:
  1. Dizziness, headache, vertigo.
  2. Vision problems.
  3. Pins and needles sensation.
  4. Tinnitus (noises in your ears).
  5. Cough and shortness of breath.
  6. Nausea, diarrhea, constipation, indigestion, taste changes, vomiting, abdominal pain
  7. headedness.
  8. Allergic reactions (such as itching and skin rash).
  9. Tiredness.
  10. Muscle cramps.

 

Pregnancy & Breastfeeding Precautions While Taking Preterax Tablets

  • If you are pregnant or breastfeeding, think you may be pregnant or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.
  • Your doctor will discuss the possible risks and benefits of using this medicine during pregnancy and breast-feeding

 

Non-Pharmacological Treatments & Advice To Reduce Blood Pressure

  • To measure blood pressure correctly:
  1. Don’t eat or drink anything 30 minutes before you take your blood pressure.
  2. Empty your bladder before your reading.
  3. Sit in a comfortable chair with your back supported for at least 5 minutes before your reading.
  4. Put both feet flat on the ground and keep your legs uncrossed.
  5. Rest your arm with the cuff on a table at chest height.
  6. Make sure the blood pressure cuff is snug but not too tight. The cuff should be against your bare skin, not over clothing.
  7. Do not talk while your blood pressure is being measured.

Warnings & Special Precautions while Taking Preterax Tablets

  • Do not take Perindopril / Indapamide:
  1. If you have an allergy to perindopril, or any other ACE inhibitor, indapamide or any other sulfonamides, or any of the other ingredients of this medicine.
  2. If you have previously experienced angioedema to perindopril or another ACE inhibitor.
  3. Angioedema is a condition that is manifested by signs and symptoms like swelling of the face, throat, or tongue, wheezing, intense itching, or severe skin rashes.
  4. If a family member has previously experienced angioedema to perindopril or another ACE Inhibitor
  • Talk to your doctor if you have hypertrophic cardiomyopathy (heart muscle problem), aortic stenosis (the main blood vessel that leads to the heart becoming narrowed), or renal artery stenosis (artery supplying blood to the kidney becomes narrow).
  • Talk to your doctor if you are suspected to have heart failure or other heart problems.
  • Talk to your doctor before starting this medicine if you have liver or kidney problems or if you are undergoing dialysis.
  • Talk to your doctor before starting this medicine if you have diabetes.
  • Talk to your doctor before starting this medicine if you have collagen vascular diseases (a disorder of the connective tissue) like scleroderma or systemic lupus erythematosus.
  • Talk to your doctor before starting this medicine if you are following a salt-restricted diet or are using salt substitutes that have potassium.
  • Talk to your doctor before starting this medicine if you are to have LDL apheresis (cholesterol removal from your blood using a machine)
  • Talk to your doctor before starting this medicine if you are due to have surgery and/or anesthesia.

  • Talk to your doctor before starting this medicine if you are intolerant to some sugars.
  • Talk to your doctor before starting this medicine if you have recently suffered from vomiting, diarrhea, or are dehydrated.
  • Talk to your doctor before starting this medicine if you are pregnant or plan on getting pregnant.
  • Tell your doctor or pharmacist if you are taking, have recently taken, or might take any other medications including over-the-counter non-prescription medicines, prescription medicines, herbal supplements, vitamins, and supplements.

 

Storage Conditions Of Preterax Tablets

  • Store below 25°c
  • Don’t refrigerate
  • Keep this medicine out of the sight and reach of children.
  • Store in the original package to protect from moisture.
  • Do not use this medicine after the expiry date which is stated on the box/blisters/label. The expiry date refers to the last day of the month.

 

Normal Ranges

  • Normal: Blood pressure numbers of less than 120/80 mm Hg
  • Elevated: is when readings consistently range from 120-129 systolic and less than 80 mm Hg diastolic.
  • Hypertension Stage 1: is when blood pressure consistently ranges from 130-139 systolic or 80-89 mm Hg diastolic.
  • Hypertension Stage 2: is when blood pressure consistently ranges at 140/90 mm Hg or higher.
  • Hypertensive crisis: This stage of high blood pressure requires medical attention. If your blood pressure readings suddenly exceed 180/120 mm Hg, wait five minutes and then test your blood pressure again. If your readings are still unusually high, contact your doctor immediately. You could be experiencing a hypertensive crisis.
