Mechanism Of Action Of Amiloride & Hydrochlorothiazide
- Amiloride hydrochloride is a potassium-sparing diuretic with mild natriuretic and diuretic properties. Its primary site of action is the distal tubule of the nephron where it selectively blocks sodium transport, thereby inhibiting sodium-potassium exchange.
- Hydrochlorothiazide inhibits sodium chloride transport in the distal convoluted tubule. More sodium is then excreted in the kidney with accompanying fluid.
- Thus it increases urine excretion, which by turn decreases blood volume and finally blood pressure.
- On long-term usage, thiazide diuretic (TZD)-mediated chronic reduction of arterial pressure is thought to occur through decreased total peripheral vascular resistance.
Therapeutic Indications Of Moduretic 50 Mg Tablets
- To treat patients with high blood pressure (hypertension): High blood pressure is a common condition in which the long-term force of the blood against your artery walls is high enough that it may eventually cause health problems, such as heart disease.
- Also, used to treat water buildup in your body (edema) due to problems with your heart, kidneys, liver, or blood vessels.
- To treat patients with heart failure: It usually occurs when the heart is unable to pump blood around the body properly.
Method Of Administration Of Moduretic 50 Mg Tablets
- Take as indicated by your doctor.
- Take with a glass of water.
- High blood pressure:
- The usual starting dose is half a tablet each day.
- Your doctor may change your dose to one tablet given each day
- Your doctor may spread out the dose or give it as a single dose
- Heart failure:
- The usual starting dose is half a tablet each day
- Your doctor may change your dose
- You should not take more than two tablets in a day
- Fluid retention due to cirrhosis:
- The usual starting dose is one tablet each day
- Your doctor may change your dose
- You should not take more than two tablets in a day
- Doses may be changed in special conditions so always follow your doctor's or pharmacist's instructions.
- If you take more than you should, talk to a doctor or go to the hospital straight away.
- If you forget to take a dose, take it as soon as you remember it unless it is nearly time for your next dose.
- Do not take a double dose (two doses at the same time) to make up for the forgotten dose.
Undesirable Side Effects Of Moduretic 50 Mg Tablets
- Purplish or reddish-brown spots, skin irritation, increased sensitivity to sunlight, and a nettle like rash (urticaria)
- Headache when you stand quickly
- Anemia: signs include unusual tiredness or loss of color in the lining of the eyes and skin around the eyes
- Other blood disorders which can result in high temperature (fever), a sore throat, being unable to stop bleeding from a cut
- Changes in the levels of various chemicals in the blood which are usually detected by blood or urine tests
- Stomach and gut
- Feeling sick (nausea) or being sick (vomiting), indigestion, diarrhea, constipation, stomach ache, stomach cramps, wind, bloated feeling, hiccups
- Inflammation of the pancreas: signs may include feeling sick (nausea) and being sick (vomiting) with
- Pain in the stomach area and back
- Swelling of the glands that make saliva
- Dry mouth
- A bad taste in your mouth
- Lack or loss of appetite
- Difficulty breathing
- Blocked nose
- Cough
- Feeling dizzy, feeling like you are spinning (vertigo), pins and needles
- Feeling sleepy, or sometimes feeling that you cannot sleep

- If you are pregnant or breastfeeding, think you may be pregnant or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.
Non-Pharmacological Treatments & Advice To Reduce Blood Pressure
- To measure blood pressure correctly:
- Don’t eat or drink anything 30 minutes before you take your blood pressure.
- Empty your bladder before your reading.
- Sit in a comfortable chair with your back supported for at least 5 minutes before your reading.
- Put both feet flat on the ground and keep your legs uncrossed.
- Rest your arm with the cuff on a table at chest height.
- Make sure the blood pressure cuff is snug but not too tight. The cuff should be against your bare skin, not over clothing.
- Do not talk while your blood pressure is being measured.

Warnings & Special Precautions while Taking Moduretic 50 Mg Tablets
- Talk to your doctor or pharmacist before taking Hydrochlorothiazide & Amiloride hydrochloride if:
- You have been told by your doctor you have high levels of uric acid, cholesterol, or triglycerides in your blood
- You are taking digitalis such as digoxin
- You are being given fluids by a tube inserted into one of your veins
- You have recently suffered from severe vomiting or diarrhea
- You have Systemic Lupus Erythematosus
- You are elderly
- You have gout
- You have had skin cancer or if you develop an unexpected skin lesion during the treatment.
- Treatment with hydrochlorothiazide, particularly long-term use with high doses, may increase the risk of some types of skin and lip cancer (non-melanoma skin cancer). Protect your skin from sun exposure and UV rays while taking Hydrochlorothiazide & Amiloride hydrochloride.
- It is important to tell your doctor or pharmacist if you take:
- Angiotensin-converting enzyme inhibitors (ACE inhibitors) such as enalapril used to treat high blood pressure or heart problems
- Angiotensin II receptor antagonists such as losartan - used to treat high blood pressure or diabetics with kidney damage
- Other medicines for high blood pressure
- Medicines for diabetes such as insulin
- Lithium: used for depression
- Non-steroidal anti-inflammatory drugs (NSAIDs), such as aspirin or ibuprofen: used to reduce a high temperature (fever), pain, or arthritis
- Barbiturates: used to help you sleep or to reduce anxiety
- Painkillers such as codeine, dihydrocodeine, dextropropoxyphene, diamorphine, morphine, pentazocine, and pethidine
- Cholestyramine and colestipol - used to treat high cholesterol (hyperlipidemia)
- Steroids - used to treat lots of different conditions such as rheumatism, arthritis, allergic conditions, skin problems, asthma, or a type of blood disorder
- ACTH to test whether your adrenal glands are working properly
- Medicines injected for allergic reactions such as adrenaline (also known as epinephrine)
- Tacrolimus: used after a liver or kidney transplant to prevent rejection
- Ciclosporin: used for rheumatoid arthritis or to prevent rejection after a transplant ‘non-depolarising’ muscle relaxants such as tubocurarine
- Trilostane: used to treat breast cancer or overactive adrenal glands (such as in Conn’s syndrome, Cushing’s syndrome)
Storage Conditions Of Moduretic 50 Mg Tablets
- Store below 25°c
- Don’t refrigerate
- Keep this medicine out of the sight and reach of children.
- Store in the original package to protect from moisture.
- Do not use this medicine after the expiry date which is stated on the box/blisters/label. The expiry date refers to the last day of the month.
Normal Range
- Normal: Blood pressure numbers of less than 120/80 mm Hg
- Elevated: is when readings consistently range from 120-129 systolic and less than 80 mm Hg diastolic.
- Hypertension Stage 1: is when blood pressure consistently ranges from 130-139 systolic or 80-89 mm Hg diastolic.
- Hypertension Stage 2: is when blood pressure consistently ranges at 140/90 mm Hg or higher.
- Hypertensive crisis: This stage of high blood pressure requires medical attention. If your blood pressure readings suddenly exceed 180/120 mm Hg, wait five minutes and then test your blood pressure again. If your readings are still unusually high, contact your doctor immediately. You could be experiencing a hypertensive crisis.
